Absence Form
Please fill out the absence form to clearly indicate your availability for any service or event. This is crucial for effective scheduling and ensuring we have the appropriate replacements ready. Leaders are also required to notify senior leadership of any absences. We appreciate your prompt attention to this matter!
Meeting space
To ensure a smooth booking process for our meeting spaces, please complete the meeting space form. This helps us avoid ministry departments double booking and ensures that all necessary equipment is available for your use. Remember, rooms should be reserved at least a week in advance to guarantee availability. Thank for your cooperation!
Reimbursement Form
Leaders are required to fill out a reimbursement form prior to any purchases. Please note that forms that are not approved may not be eligible for reimbursement. We appreciate your cooperation with this important process.
Service/Event Request Form
We require all staff to complete the forms below at least one month in advance for any special service or event. By completing the form, you help us prevent calendar conflicts, ensure the right support is informed, and allow ample time for effective event promotion.
